Ligament Injuries (ACL/MCL – Grade 1)

Ligament injuries occur when the strong bands of tissue connecting bones within a joint are stretched or torn. In the knee, the anterior cruciate ligament (ACL) and medial collateral ligament (MCL) help maintain stability during walking, running and other physical activities.

Ligament Injuries (ACL/MCL – Grade 1) Symptoms

Symptoms vary depending on the ligament affected and the severity of the injury. Grade 1 injuries are mild sprains involving stretching and minor damage to ligament fibres without significant loss of stability.

Common symptoms of Grade 1 ACL or MCL sprains include:

  • Mild knee pain: Discomfort around the knee, particularly during movement or physical activity
  • Tenderness: Sensitivity to pressure given to the affected area
  • Mild swelling: Some swelling may develop following the injury
  • Stiffness: Temporary difficulty bending or straightening the knee comfortably
  • Discomfort during activity: Pain when walking, turning, or changing direction

Symptoms of Severe Ligament Injuries

More significant ligament injuries may cause pronounced swelling, intense pain, difficulty bearing weight and a feeling that the knee is unstable or giving way. Some people hear or feel a popping sensation at the time of injury. Severe injuries may involve a partial or complete ligament tear or damage to other knee structures, including the meniscus. These symptoms require prompt medical evaluation.

Ligament Injuries (ACL/MCL – Grade 1) Causes

ACL and MCL injuries commonly occur during sports and activities involving sudden stops, rapid changes in direction, twisting, or awkward landings. An ACL injury may result from abruptly pivoting on a planted foot, while an MCL injury often occurs when force pushes the knee inward. Falls, collisions and other accidents can also damage knee ligaments.

Ligament Injuries (ACL/MCL – Grade 1) Diagnosis

Diagnosis involves reviewing the injury mechanism, symptoms, and medical history, followed by a physical examination.

  • A specialist doctor may perform specific stability tests to assess the ACL and MCL.
  • MRI may be recommended to evaluate ligament damage and identify associated meniscus or cartilage injuries.
  • X-rays may be used when a fracture is suspected.

Ligament Injuries (ACL/MCL – Grade 1) Treatment

Treatment depends on the severity of the sprain and the patient’s activity requirements. Grade 1 injuries are often managed with temporary activity modification, ice for pain and swelling, appropriate support when necessary, and progressive physiotherapy. More serious injuries may require specialist assessment and additional treatment.
