Though the terms are often used interchangeably, a strain and a sprain affect different structures in the body and require distinct treatment approaches. Understanding the differences between the two is essential for proper care and a safe, speedy recovery.

What is a Muscle Strain?

A muscle strain occurs when muscle fibres or the tendons that attach muscles to bones are overstretched or torn. This can happen suddenly, such as when lifting a heavy object improperly or during intense physical activity without warming up. Overuse injuries, where repetitive movements lead to gradual damage, are also common causes.

Muscle strains are categorised as mild, moderate, or severe depending on the extent of fibre tearing. Common symptoms include localised pain, muscle stiffness, weakness, swelling, bruising, and reduced range of motion. Strains are especially common in areas like the lower back, hamstrings, neck, and shoulders.

What is a Ligament Sprain?

A ligament sprain involves the stretching or tearing of a ligament, which is the strong connective tissue that links bones together and stabilises joints.

Sprains are usually caused by trauma such as a fall, sudden twist, or impact that forces a joint beyond its normal range. For example, ankle and knee sprains often occur during sports or slippery conditions.

Symptoms of a sprain include sharp joint pain, swelling, bruising, joint instability, and difficulty bearing weight on the affected limb. Like strains, sprains are graded based on severity; Grade I (mild), Grade II (moderate), and Grade III (complete tear).

Diagnosis and Treatment

Proper diagnosis begins with a proper medical history and physical examination. Your doctor may assess joint stability, range of motion, and muscle strength. In some cases, imaging tests such as X-rays, MRI, or ultrasound are recommended to rule out fractures or determine the extent of soft tissue damage.

Initial treatment for both sprains and strains typically includes the following:

  • Rest: Avoid activities that aggravate the injury.
  • Ice: Apply ice packs for 15-20 minutes every few hours to decrease inflammation.
  • Compression: Use bandages or braces to support the injured area.
  • Elevation: Raise the injured limb to decrease swelling.

After the initial phase, physiotherapy plays a crucial role in regaining strength, flexibility, and function. Stretching and strengthening exercises are customised based on the injury type and severity. Supportive measures such as posture correction, ergonomic modifications, and proper footwear are also essential to prevent recurrence. In severe cases, especially Grade III sprains, surgical intervention may be needed to repair torn ligaments.
